Important Update Regarding Your Medical Records & Medical Record Requests:

 

This notification serves as an important update regarding the process for accessing your medical records from Anelas Holistic Counseling PLLC.

 

As you know, our practice closed our office to continue practicing therapy and accepting patients at the end of October 2022. While your records have remained securely stored since that time, I am writing now because the process and the previous contact information is no longer valid, that were discussed at time of discharge.

 

Due to my relocation out of state of Virginia June of this year, and the subsequent changes, including me beginning the process of formally closing the practice in the State of Virginia; my contact information has been updated online via Google and the company web page.

 

Your medical records are essential to your ongoing care, and I want to ensure that you know the current, correct procedure for requesting them for yourself/others or other health/behavioral health care providers.

 

All requests of records must now be made by first calling me via telephone at 804-214-6220. In addition, please follow the steps below to ensure a timely response:

 

Obtain the Release Form: Once I receive your call, I will re-activate your patient portal and you will see a digital Release of Information available to you to fill out and submit. Please ensure the form clearly lists the name, address, phone number, and/or fax number of the individual or health/behavioral healthcare provider or healthcare facility.

 

Fees: There are no costs associated with this request. Your medical records will be provided to you through your patient portal.

 

Using the updated contact information above is required to ensure the timely fulfillment of your request.
